Service-request preparation checklist

The details worth gathering before you request elevator service, so the first visit is productive.

The problem

What is the equipment doing, in one or two plain sentences?
When did it start, and how often does it happen?
Is it constant or intermittent? Any pattern (time of day, weather, load)?
Which floors or doors are involved?
What has already been tried (resets, prior service calls)?
Photos or video taken? Where are they saved?

Urgency check

Is anyone inside a stopped car? If yes: use the in-car emergency system and call for help now; this worksheet waits.
Is the unit out of service entirely? If yes: call rather than submitting a form.
Otherwise: submit the request online with the details above.
